Management

Classification of bladder cancer that is used to guide management (simplified from NICE):

  • Non-muscle invasive bladder cancer, further categorised into:
    • High risk – carcinoma in situ / lamina propria invasion / high grade (3)
    • Non-high risk
  • Muscle-invasive

    When referring to muscle-invasive bladder cancer, the specific muscle layer of the bladder being discussed is the muscularis propria (detrusor muscle).

    Invasion into the muscularis propria distinguishes muscle-invasive bladder cancer (MIBC) from non-muscle-invasive disease, which is limited to the urothelium, lamina propria, or muscularis mucosae.

  • Metastatic disease

Exam-yield management of bladder cancer:

  • Non-muscle-invasive bladder cancer → TURBTTrans-urethral resection of bladder tumour
  • Muscle-invasive bladder cancer → neoadjuvant chemotherapy + radical cystectomy (or radiotherapy)
  • Metastatic disease → chemotherapy (cisplatin-based combination)
